About Us
At Community CORE Foundation, Inc. we understand that health in all aspects of life should be at the core of every community regardless of economic status, race, or any other factor that might increase the likelihood of healthcare disparities. That’s what drives us. It is our mission to improve health equity and quality of life in underserved communities by delivering proactive, preventative, and culturally competent care directly to where people live.
We have seen some of these inequalities and more play out in our own communities, in our own families, amongst our own friends, and even in our own lives. For us, it’s personal.
One of our solutions is screening with our CORE 8 Protocol, an evidence-based system that assesses health in 8 key areas and provides a snapshot of where those we serve are currently, at no cost to them. They will be able to track trends in their results, and will be encouraged to employ appropriate follow-up steps to maintain or improve health.

Dr. Ray is a dual-certified nurse practitioner specializing in psychiatric, mental health, and addiction treatment. What he loves about Community CORE Foundation is the way it promotes health awareness in underserved populations through education and screening.

Dr. Derrell is a chiropractor who has a passion for helping people and incredible drive to change the world around him. One of his main areas of focus is minority healthcare, and Community CORE Foundation allows him to make a difference in that arena.
